I Remember seeing one and it reminded me of Bally Games. I Think they went into fitness supplies after pinball, hence the logo similarity.

As of 2007 they were at 23303 Allen Road in Woodhaven, MI (now vacant) and in the Fairlane Town Center in Dearborn, MI (not sure what's in that space now).

I believe so - sounds like the last 3 locations of the company (all in NYC) are found via this website:
http://www.workoutnewyork.com/

Though when it opens it still has the Bally Sport name at the top of the page.

Also apparently the last non-gaming part of the company remaining, at least if our Wiki friends are correct.
