It was converted to a children's museum (top floor) and a satellite location of the Houston Museum of Natural Science on the bottom floor (again, oriented toward the younger set)

The San Antonio stores were the next-to-last to close, announcing it in Oct. 2008 and closing a few months later. They are still vacant.
North Star Mall (1992) - originally Frost Bros.
Ingram Park Mall (19??) - originally ??? (possibly original to 1979, as it existed in 1990)
South Park Mall (19??)

Windsor Park Mall also had one, which closed between 2004 and 2008. The mall is closed.

More research is needed on these

I don't know when it opened, but there was a Mervyn's in Waco, TX now a BURLINGTON COAT FACTORY.

Bonus: <a href="http://thecaldorrainbow.blogspot.com/2008/12/retro-toys-r-us-round-up.html">Vintage Toys R Us</a> is in the same center.

A few more Dallas ones:
Collin Creek Mall's Mervyns lasted from 1990-2006, was in an old Lord & Taylor and now Amazing Jake's (http://amazingjakes.com/). It's a place that seems to combine Krazy City and Chuck E Cheese's, if you get my drift.
